Primary Care Nurse Practitioner

Position: Board Certified Internal Medicine or Family Practice NP or PA
Job Type: Part Time / Per Diem
Location(s): East Hartford, Somers
Job Overview: We are seeking a qualified and motivated nurse practitioner or physician
assistant (NP or PA) to join our team. The ideal candidate will have experience in internal
medicine, family medicine, urgent care or emergency care settings. They should be
comfortable treating a wide range of acute conditions, performing minor procedures, and
collaborating with clinical staff.

Key Responsibilities:
• Deliver quality, patient-centered care to patients aged 12 and up in an outpatient
setting
• Evaluate, diagnose, and treat a wide variety of acute illnesses and injuries
• Perform minor procedures such as laceration repairs, splinting, incision, and
drainage of abscesses
• Order and interpret diagnostic tests (labs, x-rays, etc.)
• Prescribe medications and develop treatment plans for patients
• Provide patient education regarding diagnosis, treatment, and follow-up care
• Conduct thorough medical histories, physical examinations, and diagnostic
assessments
• Develop and implement individualized treatment plans, including medication
management and lifestyle counseling
• Document patient visits and treatments using the electronic medical records (EMR)
system
• Work collaboratively with physicians, medical assistants, and administrative team
members
• Provide patient education on chronic disease management, preventive care, and
health promotion

• Maintain accurate and detailed medical records, adhering to established guidelines
• Participate in quality improvement initiatives and contribute to the overall success
of the Internal Medicine Practice
• Ensure compliance with all health care regulations and company policies

• Comfortable using electronic health records and other computer systems
• Resourceful problem-solving attitude and a strong sense of accountability

Qualifications:
• NP or PA: Board-certified in Family Medicine, Emergency Medicine, or Internal
Medicine
• NP: Active NP license with certification in Family Medicine or Emergency Medicine
• PA: Active PA license with certification and experience in urgent care or emergency
medicine
• Current and active Medical License in Connecticut
• Current DEA license (for prescribing controlled substances)
• BLS and ACLS certification required; PALS certification preferred

• 1-2 years of experience in an urgent care or emergency setting is preferred but new
graduates with relevant rotations or training are welcome to apply
• Strong clinical judgment, excellent communication skills, and ability to thrive in a
fast-paced environment

• 25 years established practice

• Limited Telemedicine
• Location: In-person
